Filling Health Questionnaires
When your doctor or the system creates a health questionnaire for you, a purple card will appear on your dashboard. Follow these steps:
Tap the purple questionnaire card
The card shows the questionnaire name and days remaining
Answer each question
Choose the answer closest to how you currently feel. For "Baseline Health Assessment," recall your condition before surgery. Answers are auto-saved, so you won't lose progress if you leave.
Submit the questionnaire
After answering all questions, tap "Submit." You will see your score results, and if you have prior records, your improvement will be shown.
What is the "Baseline Health Assessment"?
This questionnaire asks you to recall your physical condition before surgery. It serves as a baseline for tracking your recovery progress. Usually completed at your first post-op visit, but can be filled later as well.
Logging Rehab Exercises
Your doctor prescribes exercises based on your surgery type and recovery phase. Log in daily to see your exercise list:
Each exercise shows its name, reps, and sets. Tap "Complete" after finishing each one.
After completing exercises, report your pain level (0-10). This helps your doctor understand your recovery.
If you cannot exercise today, you can choose to skip and provide a reason. Your doctor will be notified.
Wound Photo Upload
Regular wound photos help your doctor track healing remotely, reducing unnecessary clinic visits.
- Take close-up photos in good lighting
- Keep consistent angle and distance for comparison
- Photos are auto-compressed before upload, works even with slow connection
